"Katawa Shoujo (Disability Girls)" is an ADV-style ren'ai visual novel combining anime-style sprite/CG art with stylized photographic backgrounds, telling the story of Hisao Nakai, an aloof high school student whose life is changed by a previously unknown heart condition which leaves him weakened and requires being sent to a boarding school for the disabled. Of the things he encounters while adjusting to his new situation and school, he is struck most by five distinctive female students (Hanako, Lilly, Rin, Emi and Shizune), and as the VN progresses, he has the opportunity to find meaning in his own life through pursuing a romantic relationship with one of these handicapped women.
"Katawa Shoujo (Disability Girls)" is a romance visual novel, about a teenager, Hisao Nakai, with a congenital heart defect, who has to move to a new school, the Yamaku Academy, which specialises in enabling disabled students to live a largely normal life, in order to care for his condition.  The plot deals directly with his adjustment to a new and wildly different environment, and the love he finds there, while also tackling perceptions of those with disabilities and their treatment.

It is the freshman effort of Four Leaf Studios, a worldwide doujin circle formed in the late 2000s in response to a widely circulated illustration describing a hypothetical "crippled girl" dating sim (featuring a title and character illustrations/brief personality descriptions but no names or even a story premise). While the game strives to remain consistent to the artist's somewhat spare concept, RAITA (himself a doujin artist) is uninvolved with the actual development and prefers only to say that he looks forward to playing it once it's released.
Inspired by a concept sketch by RAITA, Katawa Shoujo is the debut production by Four Leaf Studios, a worldwide doujin circle formed to develop the concept further. The full game is currently available only in English, with translations available for the first act in Japanese, Chinese, French, German and Italian, provided mostly by outside translation teams. It is unvoiced but has an original score.

In contrast to most amateur productions, KS will have a word count/playtime consistent with most commercially produced VNs. Because of the sheer size of the project, it does not have a single scenario writer or 2-3 person staff, but instead has a separate path writer for each heroine. Character art is also divided among artists but as with the writing, this is more of a practical consideration than an aesthetic choice. Currently KS is written in American-standard English (itself an achievement as not all the writers or editors are native speakers) and contains translations to Japanese, Chinese, French, German and Italian, provided mostly by outside translation teams. It is unvoiced but has an original score.

In keeping with the expectations surrounding a project with a deliberately prurient premise, KS is an eroge with adult content ("H scenes" involving sex with the heroines) although user preferences can be set to skip these scenes.
