There are a couple of visual factors that I think makes Hanako to stand out in the cuteness department.

1. Except for missing out on big smiles (Emi has a plenty of these), Hanako has several distinct facial expressions: (1) getting startled; (2) barely staring at someone due to shyness; (3) intensely staring at someone or something; (4) anger (her bad route!) at its finest; (5) disappointment and despair (after her outburst wears off); (6) the state of drunkenness (I re-read Hanaka's route several times just to see her get drunk!). All of them have one common denominator: her big eyes and how their shapes change depending on her mood.

2. Hanako wearing an oversized pajama makes her look like a little kid. Furthermore, in the scenes involving tea party, plus Hanako's birthday party, at Lilly's room, Hanako's face is rendered with more juvenile traits. (Contrast that with Hanako in her school uniform.)
