I just finished it for the first time, I'm stunned...   :3

To the questions: It basically comes down to her feeling like a burden, like all you do for her is just to protect her, because she "needs help", instead of her being an actual a friend or potential girlfriend.

Example from the Bad Ending: "I see a therapist every week, Lily dotes on me as if I were her child and now... even you!"

1 - (Good Ending 'question') It isn't wrong that she has to decide stuff for herself, for the future and it shows you care about her and her opinions not just as someone to protect.

2 - (Neutral Ending question) - Going to her on her brithday removes the potential for the relationship to go beyond what it is at the moment because it shows her you just want to protect her. You go there on her birthday, the day she always been alone thinking by herself and everybody tells you that, "just give her some time", just to check on her, to her, it sends the message of overprotection though you still understand her a bit, shown by the fact that you took her to town with you instead of sending her to a safer place, her room.

And you end up just as a friend and protector to her, that's why she's kinda sad, because you could never be more.

"You've still got Lilly and me to help you and protect you, and once she gets back, everything will be just like she never left."

The visit in Act 3 isn't overprotection because she just had a panic attack, the worry is understandable and they also went there to give her some news, while in Act 4, again, she's just alone with her toughts, everybody even tells you that, 'she's done that, just give her some time', and you go there just to make sure she's ok, to protect her.
