An interesting idea.

One possibility would be to have an act containing flashback scenes involving Hisao and Iwanako before his heart attack.  Include some decision points which will affect the content of her letter.  Furthermore, depending on which choices Hisao has made, Iwanako could show up later.  This could cause problem if Hisao is in the process of developing a relationship with one of the girls in the Yamaku.
