You're going to want to go into your wiki's Mediawiki:Common.js or Mediawiki:Wikia.js and add the following line of code:
$('ul.tools li:first-child').after('<li><a href="http://katawa-shoujo.com/download.php">Download Katawa Shoujo</a></li>');
Replacing the link and text with whatever suits your wiki.
